Rush HQ – Waiver Form

In order to participate in Rush HQ Indoor Adventure Park recreational activities, use any of the facilities, equipment, and any other goods or services provided by Rush HQ, you must complete the Waiver linked below.

Please complete the Waiver form if you are a participant 18 years of age or older.

Parent/guardians will also need to complete the Waiver on behalf of a minor/s.
